- Application for Graduate Admissions, including a one-time, non-refundable application fee of $55.
- Official transcripts from ALL undergraduate and graduate coursework whether or not a degree was granted. Transcript forms available from the University Graduate College website.
- Official GMAT (Graduate Management Admission Test) scores.
- Foreign Students who speak English as a Second Language (ESL) must complete the TOEFL exam. A score of 587/240/95 or better is required. Foreign students must also provide a statement of adequate funding and certified English translations of transcripts. They may also be asked to take an English proficiency exam at Boise State University.

- Two completed Recommendation Forms: These forms are to be completed by a person who has been your direct supervisor in a professional setting or your professor in an academic setting.
